Job Description
JOB SUMMARY: This position has primary responsibility for intake process and case management services to clients and their families referred to Central Star Community Services.


Education and Experience

Associate Degree plus 2 years of experience in a social service field OR

Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field

License or Certification

Valid California Driver's License.


Ability to interact and communicate both verbally and in writing with clients, parents, schools, other staff, physicians, county agencies, other health care and social services providers and the public.
Ability to work independently and as a member of a team.
Ability to develop, maintain, and adjust a daily/weekly work schedule that accommodates clients’ needs and availability.
Good organizational, writing, and time management skills.
Demonstrated understanding of psychiatric illness and its treatment.
Ability to understand and work with clients from diverse cultural backgrounds in a positive manner
Demonstrated understanding and knowledge of county and community-based agencies

Ability to physically perform Community Safety Interventions (CSI)
Ability to visually and audibly assess clients’ behavior and needs.
Ability to stand and walk for long periods, up to 3 hours at a time.
Ability to drive within the community in personal vehicle

Potential for eye and wrist strain from computer screen and minor lacerations from paper cuts.

Repetitive movements- writing and typing

Blood and bodily fluid contact (category one)

Interaction with potentially assaultive clients


Quality Patient Care
Demonstrated ability to engage clients and families.
Demonstrated understanding of psychiatric illness and its treatment as appropriate for Scope of practice.
Demonstrated understanding of age related development and other issues for the population(s) served (i.e. early childhood (0-5), child/adolescent (6-15), transition age youth (16-24), adult (25-64), and older adult (65+).
Demonstrated understanding of company’s Compliance Program and Code of Ethics.
Demonstrated understanding of laws and regulation affecting behavioral health clinical care.
Clinical Documentation/EMR
Ability to perform clear and concise documentation including through the use of the EMR where applicable.
Community or Facility Safety and Risk
Ability to drive to and from client’s homes in personal vehicle, if applicable.
Demonstrated understanding of how to behave in a safe manner while seeing clients in the community or facility as applicable.
Demonstrated ability to recognize dangerous behavior by client and intervene as appropriate.

Essential Duties

Has responsibility for processing referrals:
Input and maintain referrals in referral log
Verifies MediCal/ Calviva coverage
Creates consent packets
Contacts County worker for updated demographic information
Inputs client referral information in the Electronic Medical Record (EMR) system
Has responsibility for admission process
Ensures all required forms, consents, legal documentation, etc. are in place, and that related activities have occurred in a timely manner.
Interacts with clients, families, referral sources, outside agencies, and CS staff in a professional, efficient, and quality focused manner
Provides services in the community, and in client’s homes. Uses personal vehicle to drive within the community to provide services and to transport client and/or client’s family members in personal vehicle.
Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Central Star.
Other Responsibilities

Develops and maintains relationships with external partners, including DSS and DBH
Has the ability and willingness to contribute to agency quality efforts.
Provides input to program managers on ways to streamline referral and admission operations.

We provide our services through contracts with county mental health, child welfare, and juvenile justice departments, as well as with public school districts.
