JOB SUMMARY: This position has primary responsibility for intake process and case management services to clients and their families referred to Central Star Community Services.
Education and Experience
Associate Degree plus 2 years of experience in a social service field OR
Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field
License or Certification
Valid California Driver's License.
Ability to interact and communicate both verbally and in writing with clients, parents, schools, other staff, physicians, county agencies, other health care and social services providers and the public.
Ability to work independently and as a member of a team.
Ability to develop, maintain, and adjust a daily/weekly work schedule that accommodates clients’ needs and availability.
Good organizational, writing, and time management skills.
Demonstrated understanding of psychiatric illness and its treatment.
Ability to understand and work with clients from diverse cultural backgrounds in a positive manner
Demonstrated understanding and knowledge of county and community-based agencies
Ability to physically perform Community Safety Interventions (CSI)
Ability to visually and audibly assess clients’ behavior and needs.
Ability to stand and walk for long periods, up to 3 hours at a time.
Ability to drive within the community in personal vehicle

DUTIES AND RESPONSIBILITIES:
Has responsibility for processing referrals:
Input and maintain referrals in referral log
Verifies MediCal/ Calviva coverage
Creates consent packets
Contacts County worker for updated demographic information
Inputs client referral information in the Electronic Medical Record (EMR) system
Has responsibility for admission process
Ensures all required forms, consents, legal documentation, etc. are in place, and that related activities have occurred in a timely manner.
Interacts with clients, families, referral sources, outside agencies, and CS staff in a professional, efficient, and quality focused manner
Provides services in the community, and in client’s homes. Uses personal vehicle to drive within the community to provide services and to transport client and/or client’s family members in personal vehicle.
Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Central Star.
Develops and maintains relationships with external partners, including DSS and DBH
Has the ability and willingness to contribute to agency quality efforts.
Provides input to program managers on ways to streamline referral and admission operations.

In 1988, we began with three intensive, facility-based programs — Star View Adolescent Center, Oasis Rehabilitation Center, and Starlight Rehabilitation Center. We developed each program in partnership with Los Angeles, San Bernardino, and Santa Clara Counties to serve children, adolescents, and adults with serious emotional disorders in their respective communities.
In the early 1990's, we began offering an array of in-home and school-based services to support clients in transitioning from our facilities back into their communities. We now serve thousands of clients in community-based programs provided in homes, on school campuses and in partnership with other local agencies — including full service partnerships and other support services that are funded by the Mental Health Services Act.
Today we offer a full continuum of community- and facility-based programs, as well as special education in public and nonpublic schools. We deliver a complete array of therapeutic services — including outpatient, Day Treatment Intensive, and acute and subacute inpatient treatment — all of which are grounded in clinically sound, best practices. We provide our services through contracts with county mental health, child welfare, and juvenile justice departments, as well as with public school districts.
We employ more than 1,000 behavioral health workers including psychiatrists, psychologists and medical doctors, social workers, nurses, rehab therapists, counselors, teachers, aides, and a range of paraprofessional staff.
We help several thousand children and families each year.
We offer over 30 specialized public mental health programs and a wide array of community-based support services in the following counties:
